Participate in Mediation

If you are ready to renew, please follow these steps.

  1. Complete the basic application information.

  2. If you are already participating on a LRIS Experience Panel and continue to meet the experience criteria, you do not have to fill out another experience panel application. Instead, fill out this form.
    Qualification Certification
  3. Fill out your information in our agreement.
    LRIS Agreement
  4. Mail your documents along with your check, or email your documents as attachments, and call us with your credit card.

Membership Fee

The annual membership fee for CCCBA members is $65.00 when you first apply, and on January 1st of each successive year.
